Roles and Responsibilities

  • Provides direct counseling of program participants/workers
  • Develops performance goals and behavioral criteria for participants/ workers/ with assistance of Employment Specialists/Production Supervisors in the form of an Individual Written Rehab Plan (IWRP) according to CARF standards.
  • Evaluates participant's work performance and social adaptation on a regular continuing basis, keeps individuals informed of progress and maintains records and any other documentation.
  • Monitors and reports on participant's progress with respect to readiness for placement; notifies Job Developer of participant's readiness for placement and cooperates with Job Developer with preparation for placement and job seeking skills.
  • Writes discharge summaries; quarterly reviews IEE's and any other documentation required.
  • Solicits and recruit's prospective participants for referrals and conducts intake interviews.
  • Revises and or modifies participant's rehabilitation plans and reassigns participants as-determined by progress evaluations.
  • Prepares and submits required reports to the Department of Rehabilitation Services, Department of Developmental Services and other appropriate social service agency if required.
  • Represents CW Inc. at conferences, seminars, public and private meetings relevant to program activities of Rehabilitation Division as required or requested.
  • Monitors department's statistical data with respect to goals and planning.
  • May have to attend to personal care issues of others.
  • Performs other duties as assigned.
