RN Patient Navigator, Bariatric & General Surgery, Full Time Days, Ridgewood

POSITION SUMMARY:

The RN Patient Navigator provides individualized service, navigating and supporting surgical patients throughout the health system. He/she works in a dynamic health care environment with multiple departments to ensure timely screening, diagnosis, treatment and supportive services. S/he works in conjunction with the other health care professionals to evaluate and address the needs of patients served to ensure timely access to services needed and optimize health and wellness.

EDUCATION:

RN required, BSN preferred

EXPERIENCE:
RN Patient Navigator experience preferred. Understanding of medical terminology, health care systems and health care financing preferred. Proficiency with a personal computer is necessary. Demonstrated fluency in English and Spanish.

SPECIAL SKILLS:
The RN Patient Navigator must be able to: work cooperatively and communicate effectively with a wide range of individuals, including patients and family members from diverse socioeconomic and cultural backgrounds, health care professional colleagues and external health- and service-focused organizations. Employ active listening and remain solutions-oriented in interactions with patients, families and members of the health care team. Apply knowledge of the difference in roles between clinically licensed and non-licensed professionals and act within professional boundaries. Ability to navigate the health care system in effort to obtain necessary information. Ideal candidate is self-motivated, demonstrates initiative and is extremely detail oriented. Demonstrate excellent communication, organizational, and interpersonal skills. Position may require rotating to other VMG locations from time to time when necessary.
