RHD Connecticut (CT) is looking for a Community Inclusion Specialist (CSI) to work in the area of Bridgeport and Stratford area. there are currently Full-Time and Part-Time positions available.

Position Summary

The Community Inclusion Specialist (CIS) provides assistance to adults with developmental disabilities and/or autism. The CIS creates and facilitates outings designed to promote independence when accessing the community. Other objectives are to increase community involvement, develop new skills, build social ties, and connect participants with relevant opportunities to help meet their larger goals. Participants may spend time in the community engaging in anything from cultural activities, social/recreational activities, life skills-based excursions, medical appointments, job exploration, volunteer opportunities, more. The CIS may accompany people in the community, using public transportation or in their own personal vehicle.

Essential Duties and Functions

  • Supervise and oversee participants in the community daily to achieve their personal goals take part in activities and/or events.
  • Meet the basic needs of our participants while in the community. This may include assistance with other Activities of Daily Living (ADLs) as required by the participant.
  • Implement IP's (Individual Plans) and behavior plans, as needed.
  • Maintain sufficient knowledge of participants' IPs and personal needs required to work with them in a meaningful and productive way.
  • Maintain tracking of receipts and expenditures for reconciliation.
  • Complete all required paperwork, including but not limited to case notes at the end of the visit.
  • Drive personal vehicle or use public transportation to assist individuals in accessing the community. Mileage is reimbursable.
  • Attend to basic first aid, when needed.
  • Assist with and/or provide meal preparation, light housekeeping
  • Document supports given in the electronic visit verification system

Qualifications

  • High School Diploma, Bachelor's degree preferred.
  • Three years' experience working with people with developmental disabilities.
  • Basic Computer skills using Microsoft Products.
  • Valid driver's license and clean driving record.
  • Proof of insurance with CT State minimums.
  • Subject to employment screenings including FBI clearance, if applicable.

  • Medium: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.

RHD Connecticut provides residential community living arrangements, day services, and individual rehabilitative supports to people who have developmental disabilities in the West and South regions in Connecticut. Currently RHD CT operates 10 CLAs, Continuous Residential Support programs and supports individuals who reside in apartments in the community. RHD CT operates two day programs, one in the Western region and one in the Southern region of the State.
