Responsibilities
A day in the life of a Social Worker, LCSW at Hackensack Meridian Health includes:
The LCSW provides treatment to patients utilizing a variety of modalities as a member of the clinical treatment team of the assigned unit. This position also coordinates the care and management of patients and family members as well as bringing cases to final disposition and is primarily responsible for development and implementation of social work programs in their assigned unit.
A day in the life of a Clinical Coordinator at Hackensack Meridian Health includes
The Clinical Coordinator, in collaboration with the Clinical Program Manager and specialty-specific clinical team, will identify, support and develop programs to meet the needs of complex patients and family. The Clinical Coordinator is a registered nurse who has clinical expertise to the Subspecialty providers in providing quality medical care to patients.. The Coordinator is responsible for providing professional nursing care and support to patients/families with a variety of subspecialty needs. This position will be a liaison between the family and the subspecialty team to assist in care coordination and disease specific education for patients, families, and staff.
A day in the life of a Registered Dietitian at Hackensack Meridian Health includes:
The Registered Dietitan serves as a key member of the Comprehensive Diabetes Team of the Pediatric Subspecialty Endocrinology Diabetes Team, providing general diabetes education and diabetes nutrition education in accordance to ADA guidelines. This includes completing comprehensive patient assessments, formulating a plan of care in collaboration with other disciplines, and appropriate and accurate documentation in the medical record.
Part Time - 20 hours a week: Must work Tuesday, can possibly be flexible other days
A day in the life of a Medical Office Supervisor at Hackensack Meridian Health includes:
The Medical Office Supervisor provides coverage and support, under the direction of the practice administrator and physician(s) of the practice, coordinates and supervises clerical team members and the clinical team members when assigned by the Clinical Program Manager, practice operations, and business functions including but not limited to, scheduling, registration, billing, collection, patient and team member satisfaction, and purchasing. Serves as liaison between the practice and the administration.
