Job Summary:

As a Population Health IT Architect, you will play a critical role in optimizing technology to support our organization's population health initiatives. You will be responsible for architecting, designing, implementing, and overseeing the development and testing of IT systems and solutions that contribute to improving the health outcomes of our target populations. Working closely with end users, including healthcare providers, data and application analysts, administrators, and operational stakeholders, you will ensure the efficient and effective use of technology in population health management.

Position Responsibilities:

IT Operations:

Oversee the operations, maintenance, development, documentation, building, and testing of population health IT systems. Ensure the availability, reliability, and security of Pop Health infrastructure, processes, and applications. Collaborate with end users to identify and implement operational and technical improvements, enhancements, and system upgrades.

IT Architecture:

Architect and design IT solutions aligned with population health strategies and objectives. Integrate population health IT systems with existing infrastructure, applications, and data sources. Promote interoperability and seamless data exchange between systems and stakeholders, utilizing technologies such as Healthy Planet Link, Care Everywhere, Bridges (interfaces), and FindHelp.

Care Management and Quality:

Collaborate with analysts, end users, and operational leadership to enhance care coordination, patient engagement, and care plan adherence through technology solutions. Implement systems and tools that enable end users to track and monitor care management processes, quality metrics, performance indicators, and outcomes related to population health initiatives. Work with stakeholders to stay on top of delivering quality reporting for payers and regulatory bodies (state, federal). Develop data-driven reports and dashboards to provide actionable insights for end users and facilitate continuous quality improvement.

Health Equity and Social Determinants of Health:

Collaborate with end users to ensure that population health IT systems incorporate health equity principles and address social determinants of health. Work closely with operational teams to identify and leverage data and technology solutions that improve health outcomes and reduce health disparities among underserved populations. Support end users in integrating social determinants of health data and resources into population health initiatives to address underlying social factors affecting health outcomes.

Strategic Oversight:

Provide strategic guidance and leadership on population health IT initiatives, aligning technology investments with organizational goals and objectives. Collaborate with end users to develop and execute IT strategies that optimize population health management processes and support the delivery of high-quality, patient-centered care. Stay informed about emerging trends and advancements in population health technology, health equity, and social determinants of health, evaluating their potential impact and recommending innovative solutions. Monitor and report on the performance and outcomes of population health IT initiatives, including key metrics and indicators relevant to end users.

Provide application and system-level consultation to organizational teams (application and project) to guide system solutions and technology selection, while ensuring foundational integrity.

Identifies correct sources of validated data for project or reporting needs. Comfortable working to develop, maintain and strengthen the integrity of those data sources.

Position Qualifications Required:

Required Experience:

Strong communication and collaboration skills, with the ability to work effectively with customers and colleagues from diverse backgrounds.

6-10 years of experience in Health Information Technology (HIT) roles.

Strong multi-module Epic understanding or certifications preferred, including Campaigns, Care Everywhere, Healthy Planet Link, SER, Ambulatory, Orders, MyChart, Bridges (interfaces), and Cogito, as well as familiarity with Epic's Clarity and Caboodle data models and SQL framework.

Experience in promoting interoperability and data exchange.

Solid Excel foundation and familiarity and comfort with data manipulation.

Understanding of health equity principles and social determinants of health, and experience integrating these concepts into population health IT initiatives.

Strong knowledge of healthcare data standards, regulations, change management, and data governance best practices.

Proficiency in IT infrastructure management and system administration.

Excellent analytical and problem-solving skills, with the ability to translate business requirements into technical solutions.

Ability to work independently with minimal oversight or management involvement while juggling multiple initiatives, projects, and everyday maintenance.

Project management experience is desirable.

Required Education:

B.S. in Computer Science or 8 years' experience with data management or application development.

Training / Certification / Licensure:

Epic Healthy Planet Analytics and Epic Healthy Planet Clinical Build or Compass Rose certifications preferred.
