Job Overview:

Responsible for providing individual counseling services to Center for Comprehensive Care patients.

Qualifications:

Required:

  • Masters Degree in mental health field
  • LSW or LCSW Required.
  • Experience and knowledge of HIV and community resources is required.

Preferred:

  • Bilingual strongly preferred.

Scheduling Requirements:

  • Monday thru Friday
  • Full Time

Essential Functions:

  • Provides individual counseling services to CCC patients.
  • Conducts a comprehensive initial assessment on patient.
  • Develops and implements individualized plan of care using short-term and long-term counseling plans for patients.
  • Consults with Psychiatrist and other providers regarding complex patients and regarding prescription medications prescribed by providers
  • Provide non-scheduled urgent assessment of patients with medical clinic appointments
  • Assess patients with urgent mental health conditions (e.g. suicidal, homicidal, psychotic, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities
  • Provide therapeutic interventions including crisis counseling to individuals, and groups
  • Provides education to staff on mental health issues.
  • Refers patients to necessary social and medical services.
  • Develop, implement, and maintain partnerships with other community providers
  • Responsible for maintaining up to date patient records and documenting each patient encounter.
  • Responsible for submitting and achieving target for monthly statistics.
  • Attends and actively participates in weekly clinical supervision with supervisor.
  • Complies with unit and department scheduling guidelines
  • Maintain and update professional knowledge and proficiency through continuing education, staff meetings, workshops, and serving on special committees.
  • Provide task supervision and or field instruction to master level social work students.
  • Responsible for grant reporting and data tracking pertaining to mental health outcomes.
  • Responsible for attending all grant meetings pertaining to the mental health program on site and off site.
  • Performs other duties as assigned.

Pay Range:

$29.00 minimum to $31.00 maximum Hourly Rate
