Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.
We are seeking a full-time Psychiatric Nurse to provide and assist team members in coordinating the behavioral and physical health care needs of program participants.
Additional Responsibilities include:
- Complete health assessments for participants in the program.
- Maintain a record of medical and behavioral health history and on-going medical and treatment records. Review all charts at least one time per month to ensure medical needs have been met and documentation meets applicable standards.
- Consult with and assist team members regarding development and implementation of the health care needs and goals.
- Negotiate the health care responsibilities of participants with provider agencies.
- Coordinate with team members' routine medical, vision, lab, psychiatric and dental appointments. Ensure a mechanism for reporting and follow-up on results. May accompany the participant to an appointment when it is determined by the nurse in consultation with the team, and facilitate communication between PCP (primary care physician) and psychiatrist.
- Attend team meetings regularly to review medications, emergency intervention plans, and response to treatment, and to review health care needs, participant's response to treatment and coordinate scheduling appointments.
- Monitor participant's medication. Verify insurance coverage by making phone calls to appropriate insurance and pharmacies as needed. Assist psychiatrist in completing prior authorizations if needed.
- Participate in discharge planning for participants from acute or extended care facilities or forensic settings to provide continuity of medical, substance abuse and psychiatric care.
